Rod was kind enough to share his design with me and I can attest that it works well. Only mistake I made was in using aluminum for the fabricated support because it does flex a little. I'm in the process of changing it for a steel one.
Next I ground down the flat surface needed to drill and mount the shift coupler and then drilled and tapped the hole for the bolt holding the coupler onto the shaft:
