(On a LH-drive car) When you put the pedal assembly back in position it is easy to capture one of the flexible brake lines underneath the left front corner of the pedal assembly. This happens in an area that is not visible from above nor from the footwell.
The symptom is that the lower holes will line up but the upper ones refuse to go forward far enough (because the pedal assembly is tipped back about 1/4").
The solution is simply to hold onto or tie-off the line (the lower of the two attached to the LHS of the footwell) as you are setting the pedal assembly down for the last time before fastening.
Also pay attention to the routing of the cylinder feed hoses as the run across the top of the footwell so that they don't get pinched between the pedal assembly and the body work. This is visible from above. This issue probably arises only in the case where the pedals are in the front-most position.
